Staff Health Statement

The Department of Human Services requires that employees submit their

Staff Health Statement on or before their first day of training.

A staff person or volunteer who comes into direct contact with the children or who prepares or serves food,

shall have a statement signed and dated by a licensed physician, certified registered nurse practitioner or licensed physician’s

assistant, within12 months prior to working with children or food service and every 2 years thereafter, stating that the person is free of

serious communicable disease that may be spread through casual contact or that the staff person has a serious communicable disease

that may be spread through casual contact but is able to work in the facility if specific precautions are taken

that will prevent the spread of the disease to children.

PA Child Abuse History Clearance

You will receive an email from the Lancaster County Human Resources Office detailing how to complete your PA Child Abuse History Clearance. The email will look similar to the image below.

FBI Clearance

You will receive an email from the Lancaster County Human Resources Office detailing how to complete your FBI Clearance. The email will look similar to the image below.

The completed clearance will be mailed to your home address and must be presented

to the facility’s Business Administrator on or before your first day of training in order to be

included in your personnel file.

PA Criminal Background Check (PATCH)

You will receive an email from the Lancaster County Human Resources Office detailing how to complete your PA Criminal Background Check. The email will look similar to the image below.

This completed clearance can be printed out shortly after submitting for it.

This clearance must be presented to the facility’s Business Administrator on or before your first

day of training in order to be included in your personnel file.

This completed clearance can be printed out from the account you created when you registered for this clearance.

This clearance must be presented to the facility’s Business Administrator on or before your first

day of training in order to be included in your personnel file.

The following items MUST be submitted before you can begin your new hire training:

Driver’s License

Social Security Card

3 Letters of Reference

Original College Transcripts or Copy of College Degree

(Detention new hires only)

High School Diploma, Original College Transcripts,

or Copy of College Degree

(Shelter and PULSE new hires only)

Beneficiary Information for Retirement Plan

(Name, Date of Birth, Social Security Number, and Address)
